Top answer: a face so nice you want to eat it up even though it will make you fat and give you heart attacks.

The first is a put down or aesthetic criticism, the second is praiseworthy.

The slang appears to be a phonetic portmaneau:

"Chick with a hell of a nice body, but the face is ugly.
She looked real good... but her face (butterface)" [ie, "BUT, look at her FACE!"]

The non-slang term assumes the opposite valence.

Does anyone else see how easy it is to talk past each other without useful and relevant details to know which usage is correct?

Most butterface:Slavic countries
Least butterface:Mediteranean countries.

Least butterface: Japan and Northern Europe. Japanese aren't usually well developed so the face is usually the best feature. Very unusual to see a hot body, especially one paired with an unattractive face.

Northern Europe has a lot of nice faces, well formed features but often attached to fat, sloppy bodies.
