Its good to have a healthy skepticism but what on earth leads you to believe that they have food? People do have pictures when buses take wrong turns in the countryside on their "tours" and the people in the countryside are skeletal thin.

On top of that, many of the NKers escape to China in Liaoning and Jilin. Some of the students I taught in Liaoning from the border told me that they had NKers in their family who fled the country and married rural, poor, Chinese farmers and their life was much better in China with what we would consider complete poverty. The stories they heard from those women (married into their family) were all the same as above.

What you said is true: no tattoos, family structure in place, etc. But what is the cost? If you cant divorce and starve the population, don't allow them to read books or watch TV, many things can be accomplished. Its the tradeoffs that always matter.
