I believe this is the traditional usage, though it has been used lately to mean an open affair perpetrated by the woman. The term itself derives from the cuckoo bird's laying of its egg in the nest of another bird... effectively shifting the cost of raising offspring to an innocent third party (the husband, in human terms.)
