He probably has a hernia. I used to have huge balls and I thought they gave me super powers. My doctor said I had a hernia and my intestines were spilling into my scrotum. Fuck, I always thought I just had bigger balls then other guys.

Doubt it ... Looking at his picture looks like he has a decent size abdomen and and his guts are still in there, Hernias don't get this big with out him having bowel obstruction.

Case of Elephantiasis may be .......

I couldn't help googling his condition.

It's called scrotal elephantiasis.

Basically his lymphatic drainage got cut off, causing lymph fluid to slowly but surely build up and up and up until it caused his scrotum to swell up like a balloon filling with water. It happens slowly enough that the skin just stretches out without tearing, same way skin stretches as a thin person becomes obese.

His balls remain the same size though. They're just swimming in a big pool lymph tissue.

Elephantiasis is often caused by parasitic infestation, but in the western world usually happens 'idiopathically,' which is the medical term for 'we don't know what causes this.' Just one of those little medical mysteries just aching for someone to solve it and win a nobel prize for medicine.
