My guess is his algorithm reads the pixel values then picks ones above a threshold (bright red for the lipstick, black for mascara) and removed, then filled in and blended to compliment nearest pixel values. Removing the makeup colors was probably the easy part, filling the image in with natural-looking colors is the genius part. It identifies the facial area first that's how it ignores the clothes and background.
You know the 'dropper' tool in paint, where it gives you whatever color you clicked on the image? Same thing then it uses that as an average to fill in the removed makeup areas probably.

That's one way to do it, but I think it's a little more complicated than you're giving it credit for. There's probably some machine learning and a lot of sample data involved.
