I can't answer the "by what age" question but with regards to the second question, the way your look/style is perceived by different girls changes depending on how you dress/alter your body and what girls you talk to. An average looking 19 year old can easily improve his looks in a matter of months, but "attractive" changes from girl to girl. That is to say, different body types (bulky but not fat vs. lean), ways of styling your facial hair and hair, and style will appeal to different girls. Say you're currently not getting a lot of ass, but not working out that much and are hanging out in loud clubs. If you were to pack on some muscle, throw on a v-neck tee, grow your hair out, listen to some alternative bands, and head to a hipster bar, you'd probably snag quite a few girls. You have to experiment with what type of looks appeals to different niches of girls and what gets you the greatest # of bangs and then remember what works for each type of girl and customize your game/look when you go hunting for them.
In any case improving your diet, exercise, and personal hygiene (cold showers, washing your face, always having gum, lip balm, etc.) will always make you more physically attractive
More importantly though, since you're new here be careful about posting threads on looks. We've had a lot of trolls posting "looks" threads who don't actually want advice, so if you sincerely want a few tips on this subject take the advice you get, don't complain, and don't harp too much on this as game trumps looks.

Depends. Some people are unattractive because they have bad genes. Some are unattractive because of bad presentation(for example, Plain Janes). I think the majority of unattractive people can at least bring themselves up to decent. Fix their teeth, losing weight, putting one some muscle, get a haircut that compliments their face, treat acne or scars, having a facial hair style that works with you, having a style that brings out your best. That sort of thing.
Of course even if you take all these measures and bring yourself up as high as you can, I think very few guys are going to go from being a 4 in women's eyes to being an 8. You might go from 4 to 6. Plus, women aren't as responsive to looks as men are. It's important, but women aren't walking down the street twisting their head saying "dayum" at every good looking guy that walks by. They aren't like that. So even good looking guys will never get the same attention from women that an equally good looking woman will get from men. It's just going to be a numbers game for us.
